- A statement, under penalty of perjury, that the information in the notice is accurate and that you are the copyright owner or authorized to act on their behalf.
- Your physical or electronic signature.

- Your full name, address, phone number, and email address.
- A statement under penalty of perjury that you believe the content was removed by mistake or misidentification.
- A statement consenting to the jurisdiction of the federal courts where Feedishy, LLC is located.
- Your physical or electronic signature.
7. Repeat Infringer Policy
Feedishy enforces a strict repeat‑infringer policy in compliance with the DMCA.
Repeated copyright violations may result in:
- Warnings
- Content removal
- Temporary suspension
- Permanent account termination
